Requirements vary when changing a name on an account, depending on whose name is changing and the account type.
Account owner, trustee or custodian name changes
To update your name on most account types:
- Complete the Name Change Request (PDF). You must do one of the following:
- Have your former and new name validated in section 2 of the form
- Provide a legal name change document (e.g. marriage certificate, divorce decree, legal name change certificate)
- Send us the completed form and, if applicable, a copy of the name change document, via one of the following:

Beneficiary name changes
