A new way to send forms
Based on your feedback, we’ve added a new option for submitting documents. Instead of faxing or mailing forms to us, you can now upload them to our website.
The new tool is:
- Easy to use. Upload your documents in a few simple steps.
- Secure. Files are encrypted during the submission process.
- Flexible. Upload any document that doesn’t require a signature guarantee or original certification.
To submit documents online, you must be registered on our website. If you’ve never logged in to access your account online before, you can register now.
To register, you’ll need your account number (found on any quarterly statement) and the last four digits of the primary account holder’s Social Security number.
You can submit your forms by visiting the Contact Us page and clicking Send Documents.
